As a Mom of three myself, I can honestly say homemade keepsake gifts are my absolute favorite. This is why I have created and am sharing with you this free printable Mother’s Day questionnaire for kids. This printable is a great Mother’s Day gift for kids to give to their Moms on that special day.
On this questionnaire you’ll find statements for kids to fill in their own answer such as “My Mom and I like to” and “I love her because.” There is even a spot for a drawing or photo of mom.

Keepsake gifts are great for Moms because they are good at preserving time which is something we cherish. As a mom myself I understand the importance of having that tangible item that allows us to take a trip back down memory lane and remember all the things our child was at that time because they do grow way too fast.
If you would like your child to give their Mother a special keepsake then this printable is perfect for that.
This printable is also great for any teacher who wants to give their preschoolers or elementary school aged kiddos a printable to fill out for their Moms this Mother’s Day.
My oldest daughter just started kindergarten last year and it was such a pleasant surprise when she came home with a couple Mother’s Day gifts she had made for me at school.
She was SO excited that she had a surprise to give me on Mother’s Day. I was so appreciative that her teacher had planned something like that for the kids to make and give to us Moms.
One of the gifts was a Biography of Mom. Their teacher had given them the start of three sentences that they then had to write and answer about their mom and then draw a picture of their mom.
The first sentence was “She loves” and my daughter’s response was “She loves her children.” It was such a heartwarming feeling that the first thing my daughter thought of was my love for her and her sisters.
This was so sentimental to me which is why I created this free printable Mother’s Day questionnaire so other kids could give their moms a sentimental gift just as I had received.
Supplies Needed for the Mother’s Day Questionnaire
- Printer
- 8.5 x 11 Printer Paper
- Writing Utensil
- Laminator/Laminating Sheets (Optional)
Directions
The first thing you’ll want to do is decide which free printable Mother’s Day questionnaire you want to use. I know every child has a name they prefer to call their Mom by. Whether it is Mom, Mommy, or Mama I have created a version for each of these three.
I also know that not every child has a mother or mother figure in their life. Because of this, I have created a Special Person template for those kids as well.
For each version I have both a colored and black and white template that you can choose from to print off.
If you are a teacher it could be helpful to print off copies of all four different templates and let your kids choose which one they want to use for Mother’s Day.
Once you’ve decided on which template(s) you want to use then download and print them off. To print you will need a printer and regular 8.5×11 paper. Next, get your child a writing utensil and have them fill out the free printable Mother’s Day questionnaire.
If you have a toddler who is unable to read and write yet then you’ll need to read off each question and then write down their responses. It could be helpful to take a video of them giving their responses so you can write their answer word for word. This video would be an awesome supplement to give her along with the printable.
If your kids are old enough to write and/ or read then they can fill this printable out by themselves.
Make sure they don’t forget to write their name and date at the top.
When your child is done filling out the questionnaire, they can work on the picture box. Under the box it says “Here is a picture of my Mom.” Kids can either draw a picture of their Mom using crayons or markers or they can glue an actual photo of her onto the paper.
The next step is completely optional but I HIGHLY recommend it. Once your child is finished completing the Mother’s Day printable you can laminate it in order to better preserve this keepsake for their Mom.
If you are looking for a laminator, I have this Amazon Basics 9 Inch Thermal Laminator that I use at home and I love it. You’ll also need the laminator sheets that you slide the printable into. I have used both the Amazon Laminator Sheets and the Scotch Laminating Pouches. I have yet to notice a difference between the two protector sheets so I just go with whichever one is cheaper when I need to buy them.
To laminate you just place the completed All About My Mom printable into the laminator sheet and let it slide through the laminator.

When you have finished laminating the Mother’s Day questionnaire wait until that special day and then let your child give it to her.

Not all kids have a Mother in their life so I made a Special Person version of this Mother’s Day questionnaire as well. This way, your child can create this printable for someone special in their life whoever that person may be.
If you are a teacher, you have probably had students who may not have both parents. This printable is a great way to include those students when doing Mother’s Day projects. Print off copies of all the different versions and lay them out. Then, let the students choose the version they want to use.
