I have three littles myself and every year I struggle coming up with something my daughters can make for their Dad. This year I am on top of things and have created an All About My Dad Printable.
On this printable questionnaire, you’ll find fill in the blanks statements such as “My Dad and I like to” and “I love him because.” There is even a spot for a drawing or photo of Dad.

Supplies Needed for the All About My Dad Printable
- Printer
- 8.5 x 11 Printer Paper
- Writing Utensil
- Laminator/Laminating Sheets (Optional)
Directions
Step 1
The first thing you’ll want to do is decide which All About My Dad printable you want to use. I have created six different versions of this printable for you to choose from.
- All About My Dad

- All About My Special Person
For each version I have both a colored and black and white template.
If you are a teacher it could be helpful to print off copies of all the different templates. Then, let your kids choose which one they want to use for Father’s Day.
Step 2
Once you’ve decided on the template(s), then download and print them off. To print you will need a printer and regular 8.5×11 paper.
Step 3
Next, get your child a writing utensil and have them fill out the free printable Father’s Day questionnaire.
If you have a toddler who is unable to read and write yet then you’ll need to read off each question and write down their responses. It could be helpful to take a video of them giving their responses so you can write their answer word for word. This video would be an awesome supplement to give their Dad along with the printable.
If your kids are old enough to write and/ or read then they can fill this printable out by themselves.
Step 4
When your child is done filling out the questionnaire, they can work on the picture box. Under the box it says “Here is a picture of my Dad.” Kids can either draw and color a picture of their Dad or they can glue an actual photo of him onto the paper.
Make sure they don’t forget to write their name and date at the top.

The next step is completely optional but I think it is very important. Once your child is finished completing the All About My Dad printable laminate the keepsake gift to preserve it for years to come.
If you don’t have a laminator you could also put it in a sheet protector like this.
If you are looking to get a laminator, I have an Amazon Basics Laminator that I’ve had for a few years now. It has worked great for me and I use it way more than I ever thought I would.
In addition to the laminator you’ll also need laminator sheets that you slide the printable into. I have used both the Amazon Laminator Sheets and the Scotch Laminating Pouches. I have yet to notice a difference between the two protector sheets so I just go with whichever one is cheaper when I need to buy them.
To laminate you just place the completed All About My Dad printable into the laminator sheet and slide it through the laminator.
When you have finished laminating the Father’s Day questionnaire wait until that special day and then let your child give it to him. Your child will love getting to give their Dad or special person this printable on Father’s Day!

Not all kids have a Father figure in their life so I made a Special Person version of this Father’s Day questionnaire as well. This way, your child can fill out this printable for someone special in their life, whoever that person may be.
If you are a teacher, you have probably had students who may not have both parents. This printable is a great way to include those students when doing Father’s Day projects. Print off copies of all the different versions and let the students choose the version they want to use.
