Teachers do so much for our children, and I love taking time throughout the year to show my appreciation. If you’re anything like me, you enjoy sending occasional gifts to remind your child’s teacher how valued they are.

To make gift-giving simple and meaningful, I created a Teacher’s Favorite Things Form for teachers to fill out. It takes all of the guesswork out of choosing the perfect gift and helps ensure it’s something they’ll truly enjoy.

On this form, teachers will write their favorite things from their go-to coffee order and candy to their favorite sports team and more.

Whether you are a room mom, member of the PTSO, or just a parent looking to gift your child’s teacher the perfect thing, this teacher’s favorite things form is for you!

Teacher’s Favorite Things Form

At my children’s school this year, the PTSO had all of the teachers fill out a teacher’s favorite things form before the start of the school year. The PTSO then made them accessible to parents so they could see what their child’s teacher filled out. It has been a LIFESAVER!

For this reason, I decided to make a free printable teacher’s favorite things form for other parents, room moms, PTSOs, schools, etc.

How to Use this Teacher’s Favorite Things Template

First, download and print off as many copies as you need. You’ll find the free printable near the bottom of this post. Then, have the teachers fill in their answers to the questionnaire. When they are done, make the answers accessible to parents.

What’s Included on the Teacher Questionnaire?

At the top of the form, teachers will include their name and birthday. It then asks for their favorite items such as:

  • Color
  • Soda
  • Scent
  • Hobby
  • Flower
  • Doughnut
  • Candy Bar
  • Fast Food
  • Gum Flavor
  • Restaurant
  • Salty Snack
  • Sweet Snack
  • Retail Store
  • Sports Team
  • Coffee Order

There are also a few this or that questions to help parents out.
Do they prefer books or movies, coffee or tea, chocolate or vanilla, and bright colors or neutral colors?

There is also a spot for gift card ideas. If they were given a gift card in the following amounts ($5, $20, $50), where would they like it to be?

There is another spot on the form for their classroom wishlist. Teachers can write in some items they’d like to have for their class or glue down a QR code to an Amazon wishlist if they have one.

Lastly, there is a spot for teachers to include any allergies or dietary restrictions they may have.

When To Use This Form

There is really no wrong time to send in a small gift for your child’s teacher to let them know they are valued and appreciated. Some common times people like to send in gifts are on their birthday, Christmas, and Teacher Appreciation Week.

However, sometimes the best time for teachers to get a gift is on those random Monday or Wednesday mornings throughout the year.

In my opinion, one of the best days to send in a small pick-me-up, such as the teacher’s go-to coffee order, is on the day after Halloween when they have a class full of kids who are sleep-deprived and running on sugar. Seriously, teachers are the real MVPs.
